Dr. Silvio Caruso, PH.D. is a supporting antagonist in HITMAN, serving as the main antagonist and one of the two targets (alongside Francesca De Santis) of the game's second mission, "World Of Tomorrow".
He is a famous Italian bio-engineer working for the Ether Corporation and a Providence operative.
He was voiced by Brian T. Delaney.

What Makes Him Heinous?[]
- He killed his mother in her own sleep and staged her death out of natural causes. While it was somewhat justified as she was very abusive, Silvio still enjoyed killing her and wished she would come back to life so he could kill her again.
- He was a client of the ICA who asked the assassinations of people who bullied him and whom he held grudges against, including Marco Abiatti who 47 kills during the mission "Landslide".
- He is a sociopath, as he is often rude towards his workers, causing them to dislike him.
- Under Ether's orders, he designed a virus that would kill many assassins around the world, which is based on a single bullet capable to cross a crowd of people without hurting them.
- Because of his insecurities, he wanted the virus available to anyone, like Ether wanted it available only for its leaders.
- Since he is mentally unstable, Ether was afraid of him and what he could do with the virus and wanted his superior Francesca De Santis to watch him.
- Made a virus sample with only De Santis's DNA wired to it so 47 can kill her if he throws and drops the virus at her feet
- Despite his death, the Caruso virus still haunts the world as Noel Crest planned on re-engineering the virus and tried to kill all of Providence with it which would have caused hundreds of deaths, (and would have been twisted poetry/karma that the virus The Providence Partners ordered to be made would have been their downfall).
- While most of the targets 47 and Diana Burnwood assassinate are Providence operatives, Silvio manages to stand out among the high Hitman heinous standards because Diana stated his virus was dangerous and needed to be stopped as soon as possible.
What Make Him Inconsistent?[]
- He is very tragic, as he was abused by his mother who took away his girlfriend Amelia by having their gardener's son seduce her just to tell him only a mother's love endures in order to emotionally manipulate Silvio into being dependant on her. He was also bullied repeatedly, with Marco even throwing him down a well one time, causing Silvio to seek revenge and put out a contract on him.
- Despite being abused by his mother, he still cared about her in his own way, as he tried to recreate the recipes and felt a bit of remorse for killing her.
- Because of his mother's abuse, he has moral agency issues through his PTSD and sociopathic behaviour.
- His insecurities are played for sympathy, as he had therapy sessions with Dr. Oscar Lafayette whom he explained his thoughts and he is shown to still be greatly effected by his past trauma.
- He is a well-intentioned extremist, because he thought that by unleashing his virus and making it available to everyone, he would kill the strong and free the weak from their pain.
