Inconsistently Heinous villains who suffer from psychosis. These villains are known by their extreme insanity and derangement, as well as having anger issues and are unable to hold themselves back. They are more insane than mentally ill villains, suffering from hallucinations and unpredictable behavior.

IMPORTANT: Do not use this category for any and all "mentally ill", “psychopathic”, or “dissociative” villains, because it is used for characters who display all the signs and symptoms of extreme psychosis.

Examples of these villains are Tomura Shigaraki, Jeff the Killer, Trevor Philips, Yuno Gasai, Joker, Flowey, Ash Graven, Hyness, and Danyell Radcliff.
