Inconsistently Heinous villains who exhibit unorthodox or inappropriate sexual attractions and behaviors that makes them perverted. More mischievous examples, like exhibitionism, voyeurism, or lechery may exist as long as their sexual conquests disturb others (but aren't necessarily destructive). More disturbing examples include rapists, pedophiles, zoophiles, necrophiles, and incestuous families. They may also be sexually abusive to certain partners, kidnap others for sexual purposes, pimp others, or be misogynistic/misandric.

Note: While it's more common for these villains to be featured in adult media, the more mischievous examples certainly do exist in children's media, so don't always put deviant IHs under the Mature category.
